Nanofat For Dark Circles

Nanofat for dark circles refers to the very small particles of fat that are specifically filtered and emulsified and used for areas to be  injected, in this process the harvested fat filtered and emulsified  to obtain nano-particles.This emulsion contain high percentage of stem cells,endothelial cells, monocytes, macrophages and granulocytes which activation of stem cells increases collagen production. 

The major effect of nanofat application is  tissue regeneration and rejuvenation especially  for correction of undereye circles and improvement of injured skin. During processing and filtering  harvested materials many fat cells  die (cell apoptosis) so nanofat injection do not have the capacity to build up a significant  volume.

In fact, because of the lack of adipocytes, the volumetric effect of nanofat is obviously very limited. On the other hand microfat grafting has been used to build up large volumes and tissue enhancement in the face, breasts or buttocks. Therefore, the indications for nanofat injection are different when compared with microfat grafting. Therefore, nanofat could apply dirrectly or the eyelids without risk of lumpiness and irregularity.

The main problem after nanofat transfer to under eyes are swelling, bruising, redness at injection site and some edema which may last up to 2-3 weeks to resolve.

During 48 hours after fat grafting it is recommended to cool down the area and sleep in semi sitting position for fastest recovery and stay away from blood thinning drugs such as omega 3, ibuprofen for a few weeks.When properly performed, a nanofat transfer can last a long time.

Macrofat  and microfat could transfer to the deepest tissue depth as well as periostium with blunt tipped cannula, however nanofat could apply to most superficial skin layer with sharp needle technique.

Fat injections side effects are few, rare, and minimal. Soreness and swelling are most  experienced complications. Infection presenting  may affect graft survival. Viral infections or herpes infection have also been reported at the site of cannula entry.

Fat embolism, particularly if injected with sharp needle. Cyst formation is more common when a large amount of fat is transferred.

Skin necrosis and sinus formation may result from overfilling or over augmentation of the desired site. Compression atrophy may result from over augmentation with associated avascular necrosis due to excessive pressure on vasculature which can result in arterial or venous thrombosis.

Haematoma or seroma formation may occur more commonly if blunt cannulae are not used and when the patient is on other anticoagulant medication or supplements providing an anticoagulant effect.

  1. My cheeks are beginning to sag and jowl. I’m 29, so I imagine most surgeons wouldn’t recommend a facelift, though I’m open to it. I have lines around my mouth, more fullness in my lower face, full rings of hollowness all the way around my eyes (half circle), and I can feel fat around my jaw in bumps that weren’t there a few years ago (feels like cheek fat moving down). What would you recommend? I do not want fillers. Thanks!

      You would do well with a nonsurgical, no downtime micro-needling with HIFU treatment.
      However, addressing hollowness around your eyes may require volume augmentation with fillers or fat grafting.

    Hello Doctor. I had nanofat under my eyes 8 weeks ago. The doctor has a lot of experience with it. Now I still have small white lumps under my eyes and the area is still red. I look everything but great. It looks messy. The doctor said he can suck the largest out because they are probably not going to dissolve anymore?!. He didnt give me much else of an explanation than to be patient. I dont understand why it takes me such a long time to heal? I do have 2 small children and I am sleepdeprived … . Do you think there is still a chance it will all be okay in the end? How long should i give it? Or is there a possibility my body will not dissolve the fat at all. Thank you so much for your answer.

    1. Thank you for your question and I am sorry to hear of your injection issues.
      Nanofat could not trigger immunologic or allergic reaction because it is from your own body, very compatible with the new environment.
      Nanofat processing is critical to avoid complications. If some viable adipocytes remain in the emulsion then we have microfat rather than nanofat that should not directly injected to the eyelids.
      Since nanofat is injected in the superficial dermis not the subcutaneous plane, the lumps are very easy to manage.
      I would suggest patience for a couple of weeks, gentle massaging and HIFU treatment , if the lumps persist reaching out to an expert for a consultation with surgical removal of the lumps.

  3. Hello wishing to perform a nanofat transfer to the face for acne scar and also a microneedling treatment. I would like to know is it better to do a nanofat transfer first and then microneedling or microneedling and then the nanofat transfer thank you

      We routinely perform nanofat transfer and microneedling procedure simultaneously in the same session as Nanofat needling for better outcome.
      However, we perform needling first and then micro/nanofat fat graft to avoid injury to the fat cells.

  4. Hello, I really want to get cheek fat transfer but am worried about complications. how dangerous is fat transfer in general? how likely is a lumps or oil cyst?

      Fat transfer to the face is a very safe procedure.I use precise, micro fat injection techniques that ensure maximum contact of the each graft with the surrounding, native tissues. This is ensures the highest probability of fat survival. I never inject more fat than I think the subdermal tissues can support.
      I have found that most of my patients do not experience lumps or other complications. In fact, in many cases, the fat that is transferred to the cheeks contributes to an improved and more natural-looking appearance.

  5. Hi Doc, I had fat graft to my cheeks and under eyes 3 months ago, and it’s still lumpy. Plus I now have dents and grooves where I didn’t before. Dr. took fat from my legs in office and then put it directly in my face. He didn’t centrifuge the fat.Can it be due to Dr. not processing the fat? Shall I go back and ask dissolving it? It looks horrible

      Harvested fat contains tumescent fluid, damaged fat cells as well as some oils that must be separated with processing.
      Application of this emulsion without purification may cause inflammatory reactions ,Oil cysts and subcutaneous nodules.
      In my practice after over 1000 procedures I’ve never had such complications. Having said that, dissolving with hyaluronidase is not the answer to this issue because enzyme just dissove hyaluronic acid fillers not the the fats.
      I would suggest to remove fat nodules with direct liposuction by a board certified plastic surgeon with extensive experience in fat grafting.

      Alcohol temporarily thins the blood, and increase risk of hematoma in the harvesting site, moreover heavy alcohol consumption damages liver and other organs in the body.
      Having said that, I do ask my patients not to take alcoholic drinks the day before and one week after Nanofat procedure to reduce tissue edema and decrease recovery time.

        Patients started to notice the change after nanofat injections at about 4 to 6 weeks later. Because healing process and collagen production take several weeks to complete. Having said that, the improvement was noticed for smoothness of skin (100% patients), wrinkles (40% patients), pore size reduction (15% patients), improvement in redness (10% patients)
