- Avoid alcohol for 1-2 days after injections
- Avoid vigorous exercise for 1-2 days
- Wait 24 hours before applying makeup

- Apply ice packs. The ice contracts the blood vessels and lessens the chance that your bruise will spread.

What is scar subcision?

Scar subcision is a special type of acne treatment that is best suited for those who have very deep “rolling” acne scars. Basically, after the injection of some local anesthetic, a needle is inserted under the scarred area and moved back and forth, breaking up the scar tissue. This releases the overlying skin and generally decreases the depth of the scar depressions.

Subcision vs Microneedling

It is a simple procedure, the main disadvantage being that the procedure must be repeated multiple times under each of the depressed areas that is being targeted. Scar subcision does work on “ice pick” type scars, however for these “rolling” scar depressions it is an ideal treatment.

Most people notice significant improvement; however those who experience the best results are probably those who combine this procedure with another treatment modality such as micro needling.

Both subcision and microneedling improve the appearance of scars by creating wounds in an effort to break up scar tissue and trigger collagen regrowth. Microneedling involves the use of a microneedling pen with several small needles that glide across the skin at different depths and speeds. Subcision is achieved with one larger gauge needle that is injected into scars at different angles and depths to break up scar tissue.

The mechanism of subcising deeper scar tissue also seems to be more effective than that of microneedling. It often takes fewer subcision treatments than microneedling treatments to achieve comparable improvement of depressed scars. Microneedling needles are limited to penetrating at best 2.5 mm beneath the skin surface, while subcision allows the freedom to penetrate deeper into the dermis to reach deeper dermal scars. This treatment also creates larger channels within the scar tissue, which create more space for collagen regrowth, while microneedling does not.

Indications

This technique is an excellent treatment for rolling or boxcar-type acne scars, which often have fibrous attachments holding the base of the scar down to the underlying deeper tissue. After the surrounding skin is anaesthetised, a small needle is carefully inserted under the skin to sever and release these attachments beneath the scar.

During the healing process, collagen remodelling occurs and new collagen is laid down beneath the scar, further improving the smoothness of the skin. Occasionally a filler substance (hyaluronic acid) can also be injected under the scars after the procedure, to make the scar look less visible and to produce further enhancement.

Subcision is the gold standard for treating depressed scars. When doing this procedure, the area needs to be marked and then anesthetized.The numbing is often the most difficult part as it is uncomfortable. Postoperatively bruising is the most common side effect.

Because the center portion of the indented acne scar is tethered down with collagen fibers, the filler tends to extrude around the tethered area to create a “doughnut effect.” In other words, the indentation barely improves, and the edges of the acne scar become filled and rise up higher, worsening the overall appearance.

Sunscreens come as lotions, sprays, gels, foams (such as an expanded foam lotion or whipped lotion), sticks, powders and other topical products. Sunscreens are common supplements to clothing, particularly sunglasses, sunhats and special sun protective clothing, and other forms of photoprotection (such as umbrellas).

Sunscreen types

Sunscreens are classified into inorganic (mistakenly called ‘physical’) sunscreens (i.e., zinc oxide and titanium dioxide) and organic (also, mistakenly referred to as ‘chemical’) sunscreens.

Despite popular belief, both types of sunscreens work mainly by absorption of UV light, not by reflecting or scattering it.

Medical organizations such as the American Cancer Society recommend the use of sunscreen because it aids in the prevention of squamous cell carcinomas.The routine use of sunscreens may also reduce the risk of melanoma. However, many sunscreens do not block ultraviolet A (UVA) radiation, yet protection from UVA is important for the prevention of skin cancer.

Why you should avoid sunscreen higher than SPF 50?

SPF (short for “sun protection factor”) is a measure of how well a sunscreen protects you from UVB. It seems like an SPF 100 sunscreen would give you double the protection of an SPF 50 sunscreen. But the truth is that higher-SPF products are only marginally better at shielding you from UVB, according to the Skin Cancer Foundation. SPF 30 blocks nearly 97% of UVB radiation, SPF 50 blocks about 98%, and SPF 100 blocks about 99%.

A higher SPF product might also encourage you to stay out in the sun longer. Moreover, researches indicate the higher SPF ,the more side effects due to complexity of ingredients. Instead, choose an SPF between 15 and 50, apply liberally, and reapply often.

What is russian lip filler? This new trend has introduced a brand new shape to the lips, providing a flatter, wider look to match the one supported naturally by Russian women. The trend began due to the desire for lips that are larger in volume and plumpness, without the worry of filler migration or the ‘duck lips’ and ‘trout pout’ look that many want to avoid when having lip filler. Russian vs French Technique

So what is the big difference between standard lip filler (French Technique) and Russian lip filler technique?

Typically, French lip fillers are injected horizontally into the lip, resulting in an evenly spread volume and fullness effect. However, in order to execute the Russian filler technique, your provider will opt for a smaller syringe and inject small amounts of filler vertically, focusing in on the center of the lips. Another key difference is that the Russian lip filler technique aims to heighten the lip, focused in on the center, rather than to add overall plumpness throughout the lips, thus providing that heart-shaped look.

Additionally, your Russian treatment may take longer than a standard lip filler appointment due to the necessary precision required to fill only a specific region of the lips. The procedure may last approximately between thirty minutes to an hour, and may result in additional bruising and swelling , which is completely normal and temporary. Your provider will also be able to relay an accurate timeline for the lifespan of your Russian lips, but generally your Russian lip fillers will last as long as standard lip fillers do, being anywhere from six to twelve months.

Side Effects

After receiving lip filler injections, swelling can be easily managed by applying ice to the lip area. Gently apply an ice pack wrapped in a towel or a cold compress to the lips. Be sure to use soft cloth and do not to use anything that could stick to the lips, as this could worsen the discomfort.

It is recommended that you drink plenty of water and eat lots of healthy fruits and vegetables after your procedure. Avoid very salty or spicy foods, as these can irritate the lips. Staying hydrated and eating well can help the body heal.

At night, you may wish to sleep with your head elevated to reduce blood flow and fluid retention in the area overnight. Avoid sleeping on your face.

Having said that, swelling should go down within 2–3 days after your lip filler treatment, and should subside completely within 2 weeks post-treatment.

Adrenaline Injection In Aesthetics❤

Adrenaline injection is used along with emergency medical treatment to treat life-threatening allergic reactions caused by insect bites or stings, foods, medications, latex, and other causes. Adrenaline is in a class of medications called alpha- and beta-adrenergic agonists (sympathomimetic agents)

Adrenaline should be used with caution in patients with:

• hyperthyroidism, psychoneurosis, phaeochromocytoma, narrow angle glaucoma, diabetes mellitus, hypokalaemia or hypercalcaemia.

• severe renal impairment, prostatic hypertrophy or urination difficulty

• cerebrovascular disease, organic brain damage or arteriosclerosis

• autonomic dysreflexia (hyperreflexia), particularly in spinal cord injury (e.g., tetraplegics)

• shock (other than anaphylactic shock)

• organic heart disease or cardiac dilatation (severe angina pectoris, obstructive cardiomyopathy, hypertension) as well as most patients with arrhythmias. Anginal pain may be induced when coronary insufficiency is present.

Adrenaline should be used with caution in older patients

Adrenaline should be used with extreme caution in patients with long-standing bronchial asthma and emphysema who have developed degenerative heart disease.

Adrenaline should be used cautiously, if at all, during general anaesthesia with halogenated hydrocarbon anaesthetics (See section 4.5).

Adrenaline should not be used during the second stage of labour (See Section 4.6).

Accidental intravascular injection may result in cerebral haemorrhage due to the sudden rise in blood pressure.

Tumescent anesthesia involves the injection of a very dilute local anesthetic solution, coupled with epinephrine and sodium bicarbonate, into tissue until it becomes firm and tense (tumescent). The technique was initially pioneered in the 1980s for the purposes of liposuction, but its application has since expanded to cover a variety of surgeries.

The maximum recommended dose of lidocaine, with or without epinephrine, has not historically been applied to tumescent anesthesia. The American Society of Plastic Surgeons (ASPS) has endorsed higher doses of lidocaine in tumescent anesthesia based on available data. Additional studies have suggested that liposuction itself reduces systemic lidocaine absorption, as well as peak serum lidocaine concentrations. Peak serum concentrations typically occur 13–14 hours following initial infiltration.

Interaction with other medicinal products and other forms of interaction

Sympathomimetic agents/Oxytocin:

Adrenaline should not be administered concomitantly with oxytocin or other sympathomimetic agents because of the possibility of additive effects and increased toxicity.

Alpha-adrenergic blocking agents:

Alpha-blockers such as phentolamine antagonise the vasoconstriction and hypertension effects of adrenaline. This effect may be beneficial in adrenaline overdose. (See section 4.9).

Beta-adrenergic blocking agents:

Severe hypertension and reflex bradycardia may occur with non-selective beta-blocking drugs such as propranolol, due to alpha-mediated vasoconstriction.

Beta-blockers, especially non-cardioselective agents, also antagonise the cardiac and bronchodilator effects of adrenaline. Patients with severe anaphylaxis who are taking non-cardioselective beta-blockers may not respond to adrenaline treatment.

Lidocaine maximum dosage

on the other hand, lidocaine doses between 35 mg and 55 mg/kg can be used in tumescent anesthesia administered for liposuction or nanofat grafting. Best current evidence suggests reducing this dose to 28 mg/kg for tumescent anesthesia undertaken for other procedures.¹⁹ Despite the relative excellent safety record for this technique, the same care and caution for minimizing the risk of local anesthetic toxicity (LAST) must be applied

Overdose of adrenaline injection

The most common side effects of Adrenalin include:

breathing problems, fast or pounding heartbeats, pale skin, sweating, nausea,vomiting, dizziness,weakness or tremors,throbbing headache,feeling nervous,anxious, and fearful.

After overdosage or inadvertent intravenous administration of usual intramuscular subcutaneous doses of Adrenaline, systolic and diastolic blood pressure rise sharply; venous pressure also rises. Cerebrovascular or other haemorrhages and hemiplegia may result, especially in elderly patients. Pulmonary oedema may occur.

Adrenaline overdosage causes transient bradycardia followed by tachycardia and may cause other potentially fatal cardiac arrhythmias. Kidney failure, metabolic acidosis and cold white skin may also occur.

Treatment

Because Adrenaline is rapidly inactivated in the body, treatment of acute toxicity is mainly supportive.

The pressor effects of Adrenaline may be counteracted by an immediate intravenous injection of a quick-acting alpha-adrenoreceptor blocking agent, such as 5-10 mg of phentolamine mesylate, followed by a beta-adrenoreceptor blocking agent, such as 2.5 – 5 mg of propranolol. Arrhythmias, if they occur, may be counteracted by propranolol injection.
